Background: Capsular warning syndrome (CWS) is defined as a recurrent stereotype transient lacunar without cortical symptoms. However, to date, there limited literature proposed on the definitive treatment options for prevention of persistent stroke in cases capsular syndrome. Aim: The aim this case report address dual antiplatelet therapy choice CWS prevent progressive stroke. Case presentation: A 56-year-old man with several comorbidities experienced periods limb weakness and dysarthria lasting 5 - 20 minutes complete resolution symptoms between. fluctuating episodes lasted 72 hours followed by minor gadolinium-enhanced MRI scan brain revealed right basal ganglia infarct. He was treated antiplatelet, his functional outcome favourable at 90 days post Conclusion: Dual may be beneficial therapeutic option improving outcomes early neurological deficit decreasing clinical fluctuation.
